What do I need to bring?

  •  Birth Certificate (Original or State Certified with the raised embossed seal)


  • Social Security Card (Not Laminated)


  • Parent listed on the birth certificate or legal guardian with guardianship documents


  • The Parent/Guardian must provide their valid Kentucky Driver’s License or State ID to verify identity and the applicants address if the applicant lives with the Parent/Guardian.  If the applicant does not share a residence with the parent/guardian; mail postmarked or dated within the last 90 days with the applicants name will be required. Applicants under the age of 18 MUST HAVE a parent or guardian to sign the application. Guardianship papers are required for any person not listed as a parent on the applicant’s birth certificate.

We cannot accept anything laminated or photocopied!

On January 2, 2003, Kentucky implemented a new “Child I.D.” program for applicants up to 15 years old. This program works closely with the Kentucky State Police “Missing Persons Clearinghouse” and the new Amber Alert System in the event of a missing child.

To obtain an I.D. for your child please visit either the Covington or Independence branch. To obtain a Child I.D., a Deputy Clerk will need to verify the child’s identity from their birth certificate and social security document (photocopies not accepted). The child’s parent or guardian will need to sign the application and answer the U.S. Citizen question for the child. The cost of the card is $4.00 and it will expire in four years.

The card will be in the “vertical” format since it is issued to someone under age 21. One noticeable difference on the Child I.D. is that the child’s address and signature will not appear on the face of the license or in the bar code. The address, while stored in KDLIS, does not appear on the card to protect the child in case the card is lost.
